NEW ENROLLMENT: please submit page 1 (ENROLLMENT FORM) at initial registration, along with the $150 NON-REFUNDABLE enrollment fee, page 6 (TUITION CONTRACT) and your child's most recent physical form.

CURRENT STUDENTS: PLEASE FILL OUT AND RETURN PAGES 1-6 by April 1 OF EACH YEAR, ALONG WITH THE $50 RE-ENROLLMENT FEE.
Current students need to submit physical/immunization records yearly when the one on file expires. You do not need to submit one again with re-enrollment.
If your child has medication you will be leaving here, we will need to have an Individual Health Care Plan on file. Please email Deija at firstname.lastname@example.org if this applies to you so we can get that going. Thanks so much!
